In the same document, you may also appoint a <strong>Healthcare Proxy<\/strong>.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Role of the Healthcare Proxy (PCS)<\/strong><\/h3>\n\n\n\n<p><br>The Healthcare Proxy is the person authorised to make decisions on your behalf when a clinical situation arises, or simply to ensure that your Living Will is respected, provided it is sufficiently clear.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>What do you need to complete a Living Will?<\/strong><\/h3>\n\n\n\n<p><\/p>\n\n\n\n<p><br>You will need your NHS User Number (<em>N\u00famero de Utente<\/em>) and the <a href=\"https:\/\/servicos.min-saude.pt\/utente\/Repo\/feeds\/files\/Rentev_form_v0.5.pdf\" target=\"_blank\" rel=\"noopener\">official form <\/a>(available online via the health services portal).<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Registration and validation<\/strong><\/h3>\n\n\n\n<p><\/p>\n\n\n\n<p><br>Once completed, the Living Will must be registered on the <a href=\"https:\/\/www.sns24.gov.pt\/pt\/inicio\" target=\"_blank\" rel=\"noopener\"><strong>RENTEV<\/strong> <\/a>portal, where it is reviewed and submitted for validation. After validation, it becomes accessible on a secure platform so that healthcare professionals can consult it when necessary. Alternatively, registration can be done in person at designated counters or by post, in which case your signature must be notarised by a lawyer or notary.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Who can make a Living Will?<\/strong><\/h3>\n\n\n\n<p><\/p>\n\n\n\n<p><br>Any adult with legal capacity who is not placed under guardianship due to mental disability, and who is resident in Portugal (national, foreign, or stateless), may register a Living Will.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Validity and cost<\/strong><\/h3>\n\n\n\n<p><\/p>\n\n\n\n<p><br>A Living Will is valid for <strong>five years<\/strong>. Sixty days before expiry, you will be notified and may confirm that you wish to maintain the same directive or file a new one. It may be revoked or annulled at any time using the same channel used for registration or via the <a href=\"https:\/\/www.sns24.gov.pt\/pt\/inicio\" target=\"_blank\" rel=\"noopener\">Patient Portal<\/a> (<em>Portal do Utente<\/em>).
